Waiver and Liability Form
This agreement releases “Song of the Heart” Healing Arts for Transformation, from all liability relating to injuries that may occur at the Copper Harbor Retreat Copper Harbor, MI. (9-7-16 – 9-10-16) By signing this agreement, I agree to hold, Song of the Heart” Healing Arts for Transformation, entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved by participating in this retreat. These include, but are not limited to, workshop, such as experiencing deep Transformational Meditations/Activational Shifts, which may be experienced as Physical, Emotional, Mental and Spiritual or (all of the above) sensations. Also inclusive are all optional activities, including Hikes, campfires, and Light House Tours including of any and all Transportation or ride share (if applicable) to these optional events, and other activities.
I certify that I am participating voluntarily, and that all risks have been made clear to me.
Additionally, I do not have any physical or mental conditions that will increase my likelihood of experiencing injuries while engaging in the activities included in the Song of the Heart” Healing Arts for Transformation, Copper Harbor Retreat.
By signing below I forfeit all right to bring a suit against Song of the Heart” Healing Arts for Transformation, Copper Harbor Retreat for any reason. I will also make every effort to obey safety precautions as listed in writing to follow upon registering about what to bring etc, as well as what is explained to me verbally by the retreat organizers. I accept responsibility for asking for clarification when needed.
